PLAN Connections

Connections is the newest initiative of PLAN, a program of JFS Dallas. It provides comprehensive, community-based support for individuals with mental health challenges and their families.  The PLAN Connections team offers free Peer Support services to those who can benefit, regardless of PLAN membership status. Case management services are also available to PLAN members. These programs are generously funded by the North Texas Behavioral Health Authority. 

Our designated Case Manager supports PLAN members with housing, benefits, healthcare, community resources, and daily living skills. This role also handles intakes for the Clubhouse and Connections unit. Supportive counseling is offered through strengths-based, solution-focused conversations, and advocacy is provided to empower members, strengthen autonomy in their choices, and help them feel supported. 

Our team of Mental Health Peer Specialists uses their lived experience to help those who are struggling with emotional support and goal setting.

How to Access Services for Peer Support

Step 1:

Contact our team to register

You can make your request or inquiries for services online or over the phone. This initial intake process helps our team understand your needs and match you with the services that can support you best.

If you or a family member is in immediate danger, call 911.

Leave a message on our confidential voicemail with name, phone number, and type of service requested.

Step 2:

Make a plan

After we receive your inquiry, our team will reach out to you via phone call or email to discuss specific details and next steps in 1-2 business days. Together, we’ll make a care plan that works for you.

Step 3:

Start receiving care

Whether it’s a medical evaluation, a counseling session, or a meal delivery, you can start receiving quality care from our trusted network of partners and professionals.

  • Do I have to be a PLAN Clubhouse member to receive peer support?

    You do not have to be a Clubhouse member! We offer free Peer Support and Case Management services to those who can benefit, regardless of PLAN membership status.

  • What is the difference between a case manager and a Peer Support Specialist?

    A case manager focuses on coordinating services, resources, and practical supports to help someone meet daily needs and goals, while a peer support specialist provides emotional support and encouragement grounded in their own lived experience with mental health recovery.
