Our community-based clinic offers access to primary healthcare for the entire family, ensuring that everyone receives the care they need.
Income-based discounts are available, so cost is never a barrier to high-quality care.

Heather Esquivel, MD
Chief Medical Officer
Dr. Esquivel is a board-certified Family Medicine physician. As part of her patient-centered philosophy of practice, she makes every effort to understand the elements in a patient’s life that contribute to their health.
She is passionate about preventive medicine, chronic disease management, women’s health, and adolescent medicine. Dr. Esquivel attended Brown University, where she earned her bachelor’s degree in Public Health and her medical degree through Brown’s Program in Liberal Medical Education.
Throughout her career, Dr. Esquivel has been dedicated to providing high-quality healthcare to underserved communities. She has served as both a treating physician and Chief Medical Officer at Federally Qualified Health Centers around the country. As a National Health Scholar Corp recipient, she is committed to improving access to healthcare and promoting wellness. She is also bilingual in English and Spanish.
Outside of her medical practice, Dr. Esquivel enjoys exploring the arts and engaging in wellness activities such as yoga and Pilates.

Stacy Tomberlin, DNP, APRN, FNP
Family Nurse Practitioner
Stacy Tomberlin has a clinical doctorate and masters as a nurse practitioner. Stacy has been a nurse practitioner for 19 years, the past 9 years which she spent in internal medicine at Texas Health Physicians Group (THPG). While there, Stacy was honored as the top THPG provider as ranked by CGCAAPS scores earning the 2017 Culture of Excellence Award.
Before moving to THPG, Stacy worked in allergy and immunology, family medicine, employer-sponsored primary care clinics, community medicine and neurology. In receiving her Doctorate of Nursing Practice, Stacy was honored with several awards including the highest academic performance in her class. In 2014, Stacywas also selected as 1 of only 18 nurse practitioners in the United States to participate in the first year-long training program of Future Leaders of the American Academy of Nurse Practitioners.
As a healthcare provider, Stacy enjoys partnering with patients to provide the highest level of care to their unique needs. Whether you need preventative, acute or chronic disease management, Stacy will work to ensure you understand your best options to maintain your health.
